For immediate release:  All Minors Must Appear in Person When Applying for U.S. Passports

On February 1, 2008, the U.S. Department of State's Bureau of Consular Affairs begins requiring the personal appearance of all minors applying for U.S. Passports.  This includes children under age 14, who were not previously required to appear.  The change is being implemented to enhance accurate identification of applicants and aid in the prevention of international child abduction trafficking.


FIRST PASSPORT OR ISSUED OVER 12 YEARS AGO (ADULT OR CHILD UNDER 18)
2. Birth Certificate: Original or Certified Copy, must contain the following info:
A. Name of Applicant
B. Date of Birth
C. Place of Birth
D. File of Registration Date
E. Certification (Seal)
3. Identification: Acceptable documentary evidence of identity (In most cases a drivers license is enough)
4. Two (2) Passport Photos: Identical, 2 x 2, full face, plain light background
5. Appropriate Fees: (be prepared to write two checks - one to passport services the other to the Town of Standish)
        a. PASSPORT BOOK - Adult, 16 years & older, $100.00 ($75.00 for passport and $25.00 for execution fee)
        b. PASSPORT CARD - Adult, 16 years & older, $45.00 ($20.00 for passport and $25.00 for execution fee**)
        c. PASSPORT BOOK -  Minor, under 16, $85.00 ($60.00 for passport and $25.00 for execution fee)
        d. PASSPORT CARD -  Minor, under 16, $35.00 ($10.00 for passport and $25.00 for execution fee)

** The $25 Execution Fee is not charged when adults 16 and over with fuly vaild passport books apply for the passport card using a DS-82 form.

RENEWAL: You are qualified for this if the old passport was issued:
A. Within the past 12 years for the date of issuance
B. After their 16th birthday
C. You can submit that most recent passport
D. You are using the same name or can submit the marriage certificate or court order to show the name change

If you fulfill all of the above, submit an Application for a Passport by Mail, Form DSP-82, along with two new photos, your old passport and $67.00 by check or money order to the address listed on the reverse of the application.

GENERAL INFORMATION:
1. Normal processing is within 25 workings days (5 weeks) from the date of receipt.
2. Passports are accepted Mondays through Friday from 9:00 am to 4:00 pm.
3. If you have lost your passport you must apply for a new one, not a renewal.
4. Only one person may be on a passport. Each child must have their own passport.
5. Passports are valid for 10 years for an adult (16 and over) and 5 years for a minor.
6. A child needs to appear in person if they are 14 years old or older. Parents' or legal guardian(s) must accompany them to cosign for them. Both parents' or legal guardian(s) must sign for children under the age of 16.
7. All old passports are cancelled and returned to the customer with the new passports

The Town of Standish acts as an accepting agency to facilitate processing. We do not issue passports nor can we tract them. FMI call the National Passport Information Center 1-900-225-5674 (or for TDD: 1-900-225-7778)
