How Do I Apply For a Passport

The Town of Standish is a full-service Passport Agency. Our schedule for passports is Monday through Thursday from 7:30 am until 4:30 pm.  To apply for a passport you need to bring the following:  A DS-11 form used for new passport applicants (adults and children).  We have the forms here, or you can obtain them from the Department of State's website. YOU MUST COME IN WITH A 2x2 PASSPORT PHOTO.  As of OCT. 1, 2016; GLASSES CANNOT BE WORN IN THE PICTURE and WILL NOT be accepted (CVS, Walgreen's and Walmart etc. do passport photos); an original birth certificate (must have raised seal with both parent's names listed); a driver's license or State ID; and a check or money order (cash and credit cards are not accepted as payment for the passport).  The cost for a passport book for adults is $130.00 (age 16 and up) and is valid for ten years.  If a child is 16 years of age and doesn't have a driver's license, a parent must be present to identify them and sign the application.  The cost for children (age 15 and under) is $100. 00 and is valid for five years.  If the passport being applied for is a child under the age of 16, BOTH parents MUST be present unless one has a signed notorized affidavit as to why they cannot be present. Also a copy of the absent parent's drivers license (front and back) must accompany the affidavit.  The cost for an adult passport card is $30.00 and the cost for children under age 16 is $15.00.  Expedited passports have an additional $60.00 fee per passport.  There is also a $35.00 passport execution fee per passport which is payable to the Town of Standish.  Renewal of passports can be done directly by the applicant.  You need a new 2X2 passport photo and you can submit the application yourself (instructions are on the form). The current passport you have must go with the application.  The fee for the renewal passport is the same as a new passport ($130 age 16 and up and $100 for 15 and under); a check or money order must accompany the application.

UPDATE AS OF APRIL 2,2021....Routine passports are taking 10-12 weeks to process and Expeditated passports are taking 4-6 weeks. If you wish to have your passport in 4-6 weeks, you are urged to pay the expeditated fee per U.S. Department of State Passport Services
